**The Role | Mo tenei turanga mahi**
**0.5 FTE, on a fortnightly pattern of Tuesday and Wednesday one week, and a Tuesday, Wednesday and Friday the other week from 8.30am-5pm, based in Golden Bay.**
The Access and Choice service provides free health and wellbeing support to people registered with their GP practice. Based in the Golden Bay Community Health Centre, in Takaka, the health coach works collaboratively with health professionals at the practice to improve health and wellbeing outcomes on a wide range of presenting issues particularly where lifestyle change can make the difference.
In this role, you will:- Work across the adult lifespan with those aged 18 years and over.
- Collaborate with and serve as a resource person for the general practice team.
- Demonstrate strong cultural awareness to effectively support our diverse community
You:
- You must be eligible to work in NZ, which includes having NZ residency or a valid NZ work permit.
- Will hold level 4 New Zealand Certificate in Health and Wellbeing (NZCHW) or a recognised equivalent.
- Will have experience as a health professional or a Support Worker
- Must pass a satisfactory police check as part of the employment process.
- Will hold a full, New Zealand driver's licence.
