Previous experience in ID is highly desiredAbout the roleThe role sits in the IntellectualDisability liaison team based within the community ID service. The role of the CareManager is to act as the clinical manager for Care Recipients (under IDCC&RAct), to direct and oversee their care with appropriate consultation and inputfrom other health workers while the care recipient is detained within hospitallevel services. Alongside the day to dayaspects of overseeing the care of the care recipient, the care manager isrequired to develop robust care and rehabilitation plans and to ensure these are adhered to. They will liaisewith identified people to ensure the care and rehabilitation plan remains aliving document and its content evolves consistent with the needs of the carerecipient and the recommendations of Specialist Assessors when reviewed everysix months. This Care Manager role alsoincludes contributing to specialist assessments and creating Care &Rehabilitation Plans for proposed care recipients in the community.About the Team/Service/LocationThe Intellectual Disability Liaison Team is part ofthe Intellectual disabled Persons Health Service (IDPH). The wider service provides services foradults across community and inpatient settings within their forensic and dualdiagnosis teams. IDPH is a dynamic and supportive workplace whereautonomous practice, research, and professional development are supported.You will be joining a multi-disciplinary team ofhealth professionals committed to the delivery of gold standard care andrehabilitation to the population we serve. We are a progressive service committed to providing aculture of ongoing professional development for staff through regular clinicalsupervision, excellent professional development, regular in-service training and support towards externaleducation.About YouIn this role, you will be responsible for:The care manager will also developand maintain strong relationships with the Forensic Co-ordination Service (ID) and other Care Managers/ID services across themotu.A significant component of thisrole will also be case management of RIDSAS clients (Care Recipients living inRegional ID Supported Accommodation Services). This requires a senior clinician toprovide consult and liaison and specialised clinical input into the MDT teamand NGOs for eligible consumers particularly around risk management. Skills and experience Significant community and/or inpatientexperience working with tangata whaiora with an Intellectual Disability andbehaviours that challenge in either ID/Mental Health/ and/or Forensic services.Ability to work in a complex and demandingenvironment and being able to meet the needs of multiple stakeholders at thesame time.Current full drivers licence for both manual andautomatic vehicles. The knowledge required by a CareManager to manage the job appropriately can be seen as covering threeoverlapping areas.These are:An understanding of the legal framework withinwhich they operate. An understanding of the relevant legalprocesses.
