Needs Assessor / Service Coordinator - Wanganui

About the Role
Supportlinks is the Needs Assessment and Service Coordination (NASC) service which
provides access to support services in the Te Pae Hauora o Ruahine o Tararua MidCentral district for people with acute/short term needs, people who have long term health conditions, and older people/kaumatua with age related health conditions.
In this role, you will be part of the OPAL Community Services team, working across the MidCentral District, alongside a supportive transdisciplinary team consisting of geriatricians, occupational therapists, physiotherapists, social worker, Maori connectors, allied health assistants.
Supportlinks currently has a permanent full time (1 FTE) position which is based in our Palmerston North office. We are looking for a person who is enthusiastic, energetic, resilient, and has a "can do" attitude, to join our supportive team and would welcome applications from candidates who hold an annual practicing certificate in any of the following disciplines; Nursing, Occupational Therapy, Physiotherapy or Social Work.

About you- an ability to work within a changing health environment
- a mana enhancing and restorative approach within your practice
- Experience and ability to work effectively within a trans-disciplinary team, utilising individual and professional skills.
- knowledge and working relationships with iwi/Maori providers in the district
- knowledge of community resources and networks
- skills to use a laptop and learn new assessment software
- a strong desire to help clients and whanau achieve their goals
- an ability to communicate with people from a wide variety of backgrounds and cultures
- effective written, oral and interpersonal skills
- strong organisational and time management skills
- a full, current, driver's licence
