- Hillmorton Hospital, Wahi Tautoko TeamPermanent part-time position working 37.5 hours per weekPrevious experience in mental health desirable Health New Zealand | Te Whatu Ora is firmly grounded in theprinciples of Te Tiriti o Waitangi and is committed to building a health systemthat serves all New Zealanders.About the roleWe have an excellent opportunity for you to work in a newservice within Specialist Mental Health Services (SMHS) at Hillmorton Hospital.Wāhi Tautoko is a 10-bedded service located in Building 8b.We aim to support consumers that require extended assessment or treatmentfollowing an admission to the acute inpatient service (Te Awakura) prior todischarge.
- Morning, Afternoonand Night shifts, Monday to Sunday.This may include:tāngata whaiora who are presenting to services for the firsttime that may require extended assessment and support around management oftheir symptoms.tāngata whaiora that have an identified discharge pathway onadmission
- i.e. home or supported accommodation
- but may require someadditional assessment and upskilling to safely and successfully transition intothe community.
tāngata whaiora who may have limited social supports andrequire support to increase their engagement in their local communities.
The goals of the service include:Provision of a safe and supportive environment to provideextended assessment, treatment and rehabilitation following an admission to theadult acute service.Teaching skills to help support illness management and tosupport consumers to live the life they want to live.Support development of structure and meaningful routines.Support safe discharge into a community setting.
About You The role of the Healthcare Assistant offers an excitingopportunity for a dedicated and motivated individual.You will find yourselfworking alongside tāngata whaiora and their whanau, enabling them to achieve theirpersonal goals and aspirations.
You will be an integral part of themulti-disciplinary team at Whai Tautoko, with strong links to the wider SMHSand the community.
