You will work with people who are navigating physical health challenges, helping them achieve wellness goals and maintain independence in their own environments.This role is based out of our central Tauranga office and covers the wider Western Bay of Plenty region.
As part of a multidisciplinary team, you will assess client needs, contribute to discharge planning, and collaborate across sectors to ensure comprehensive, coordinated care.Your work will involve forming strong community networks, advocating for clients, and supporting them to access the health and social services they need.
The position is a casual role working ‘as and when required'.About the Team/Service/ LocationOur CAH team includes Allied Health Assistants, Dietitians, Occupational Therapists, Physiotherapists, Social Workers, and Speech-Language Therapists.

You enjoy working autonomously, but you are also a team player who brings great communication and connection building skills. You have a sound understanding of the health system and are comfortable navigating services by advocating for your clients.What you will bring: Minimum of 3 years' experience as a Social Worker in a health-related settingA current Annual Practicing Certificate and registration with the NZ Social Workers Registration BoardA full NZ driver's licenseConfidence managing a varied caseload and working across a wide geographic areaProven time management and interprofessional collaboration skillsExperience working in community, physical health, or rehabilitation settings (preferred but not essential)Strong cultural awareness and commitment to equitable service delivery Working for Health New ZealandHealth New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
