About the Role:We’re seeking someone who is passionate about delivering high-quality care with a strong focus on chronic disease management and preventive health, especially in the areas of Cardiovascular Disease and Diabetes.
You’ll work collaboratively alongside our dedicated GPs and experienced nursing team, all backed by our fabulous reception and admin crew. This is a 0.8 FTE role with flexibility for the right candidate.
What We’re Looking For:- A registered Nurse Practitioner with experience in primary care.
- Passionate about chronic care management and improving long-term health outcomes in our patient community.
- Confident running clinics independently with a proactive, patient-centred approach.
- Skilled in prescribing and medication management aligned with current best practice.
