These casual positions offers a unique opportunity to work in an innovative and evolving model of care.You will provide comprehensive nursing assessments for patients deemed suitable for Hospital in the Home by a senior medical officer. Using telehealth, you will assess health history, clinical observations, diagnostic results, and identify patient needs, risks, and early warning signs that inform care planning and required interventions.
You will take a proactive approach to patient management, ensuring timely escalation and coordination of care as required.About the Team/Service/ LocationThe Hospital in the Home (HiTH) service operates as a virtual ward, providing seven-day coverage from 7.00 am to 10.00 pm.
