Our main sites are Middlemore Hospital, Kidz First Hospital and Manukau Health Park.We are part of Health New Zealand- Te Whatu Ora, the overarching organisation for New Zealand’s national health service.

About the role:Need a Change?Then Needs Assessment could be for you!Registered Health Professional Specialising in Needs Assessment and Service coordination for Older People and People with Chronic Health Conditions, 0.8 FTE permanent or 1.0 FTE Fixed term for 12 months within the Community and Specialised Health Services, Health New Zealand, Te Whatu Ora, Counties Manukau District.We are seeking an experienced, client-focussed health professional to join our dynamic needs assessment team.
We have a permanent 0.8 FTE role available and a 1.0 FTE Fixed-term role available, working within our acute inpatient wards or the community NASC role.This role will be flexible and involve assessing people within the hospital or as required in their homes within the community from bases in Pukekohe, Botany, Papakura and the Middlemore site.This essential role, forms part of the interdisciplinary team and involves assessing the needs of adults experiencing health issues.
