This role provides assessment, treatment, monitoring, and coordination of care for individuals experiencing mental health crisis, with a strong focus on keeping people connected to their whānau and community.Working as part of an urgent response team, you will deliver intensive short-term support that offers an alternative to inpatient admission, supports early discharge, and helps prevent escalation into secondary services.The role requires strong clinical judgement, the ability to respond quickly, and a focus on safety for both individuals and the wider community.This is a dynamic and evolving service where your input will contribute to shaping how care is delivered within the community setting.About the Team/Service/LocationThe Home Based Treatment Team sits within the Adult Community Mental Health and Addictions Acute Care Service based in Whakatāne.
This newly created team works alongside existing community and inpatient services to provide a more flexible and responsive model of care. The focus is on delivering timely, intensive support that enables recovery within familiar environments.
You will be part of a multidisciplinary service committed to improving access, reducing hospital admissions, and strengthening outcomes for people and their whānau. You are confident working both independently and within a team, and you bring a calm, practical approach to crisis situations.
