As a Rehabilitation Assistant or Healthcare Assistant, you will work under the delegation of registered health professionals to deliver individualised care and therapy in clients homes. This role is pivotal in enabling patients to recover faster, improve their functional resilience, and seamlessly reintegrate into their primary care routines.About the Team/Service/LocationThe START team is a multidisciplinary group of dedicated professionals committed to enhancing patient outcomes by providing rapid and effective support to older people transitioning from hospital to home.

You are adaptable, organised, and confident working both independently and as part of a multidisciplinary team.
