About the RoleThis position is open exclusively to Health New Zealand | Te Whatu Ora staff only.An exciting opportunity has arisen for Registered Nurses to join our new Hospital in the Home (HITH) team, known as Home Ward Waikato. You will be part of a highly skilled multidisciplinary service, working alongside experienced clinicians with strong SMO and Nursing collegial support.The Hospital in the Homeservice providesacute care within a persons own home or preferred non-hospital location.

We deliver the same high standards of hospital-level treatment including access to advanced technologies, equipment and monitoring while ensuring safety, quality, and patient-centred care remain at the forefront.Our service empowers patients and their whānau by providing choice, dignity, and the ability to receive care aligned with their values, needs, and aspirations.In this role, you will play a key part in supporting effective and efficient acute flow, supporting patient transitions primarily between Waikato Hospital into HITH.

You will work closely with a range of clinicians to facilitate, promote, and coordinate safe admission into the service while ensuring continuity of high-quality care outside the hospital setting.We require a dynamic Registered Nurse to be responsible for the coordination, assessment, planning, and delegation of patient care.
Although this position is based in Hamilton, the RN will be required to travel within the Health NZ Waikato district. Fleet vehicle provided. The salary for this position will be based on experience and in line with the NZNO Collective agreement.

About the ServiceThe new HiTH service will be delivered as a collaborative service between general medicine and the community START service, promoting a `home first approach to acute care, allowing care and monitoring to be delivered safely within their own home or chosen environment.The model for HiTH has been developed with a view to reducing the pressure on hospital beds, through promoting hospital avoidance, earlier discharge and allowing patients to receive care at home where safe to do so.

Equally significant is the impact that such a model has on improving the quality of care for this group of patients. The team will consist of General Medicine Consultants (located at Waikato Hospital) and Registered Nurses (based in the community) working collaboratively across the region with the support of remote monitoring and mobile devices.

About youWe are seeking a confident and experienced senior nurse who is passionate about providing acute care in the community and supporting patients to safely transition home.To be successful in this role, you will demonstrate:Acute care experience within a hospital and/or community settingProven clinical assessment and decision-making skills in managing complex patientsStrong communication and collaboration skills, working effectively across multidisciplinary teamsCommitment to equitable, whānau-centred care, and partnership with patients and their familiesAbility to work autonomously, with clinical reasoning and problem-solving skillsA current New Zealand Nursing Council registration with a current Annual Practising CertificateA current Full Drivers LicenceExperience working in hospital avoidance, early supported discharge, or outreach programmes will be an advantage How to ApplyTo apply please click Apply online or apply via our Waikato Careers website by clicking here.
