About the role:Need a Change?Then Needs Assessment could be for you!Registered Health Professional Specialising in Needs Assessment and Service coordination for Older People and People with Chronic Health Conditions, 0.6 FTE Fixed –term for 6 months with Community and Specialised Health Services, Health New Zealand, Te Whatu Ora, Counties Manukau District.
We are seeking an experienced, client-focussed health professional to join our dynamic needs assessment team.We have a fixed-term role at 0.6 FTE available, working within our acute inpatient wards.
This role will be flexible and involve assessing people mainly within the hospital or as required in their homes within the community from bases in Pukekohe, Botany, Papakura and the Middlemore site.
This essential role, forms part of the interdisciplinary team and involves assessing the needs of adults experiencing health issues.
You will be working with various team members in partnership with clients and whaanau.
You will be carrying out specialised assessments using the internationally standardised InterRAI assessment tool and putting care plans in place, to ensure that clients are supported to return to their own homes, residential care facilities or other community living settings.
