Care Manager - Community Support Services

About the Role

We have an exciting opportunity for a motivated and compassionate health professional with a passion for working with older people, palliative clients, and those living with chronic health conditions. As a Care Manager within the Community Services Group, you will carry out comprehensive assessments, develop person-centred care plans in partnership with whanau, and coordinate support packages to ensure individual needs are met and regularly reviewed.

This role involves working closely with service users and their whanau to promote self-management wherever possible, while also collaborating with hospital-based clinicians, community services, and other providers to deliver seamless, wraparound support.

You'll need to be relationship-focused, organised, and detail-oriented, with the flexibility to operate across a range of settings, including people's homes, hospitals, hospices, and residential care.

Based at either Taranaki Base Hospital or Hawera Hospital, there is also scope for some remote working. A full NZ driver's licence is essential due to the community-based nature of this role.

About the Service

The Community Services Group at Health New Zealand | Te Whatu Ora Taranaki plays a key role in supporting people with complex health needs to live safely and independently in the community. Our multidisciplinary team includes nurses, allied health professionals, and care managers who work collaboratively to deliver coordinated, person-centred care.

We support a wide range of clients across the Taranaki region, with a strong focus on older adults, palliative care, and long-term condition management. Our service is built on principles of manaakitanga, whanaungatanga and equity—ensuring each person's dignity, preferences and cultural needs are respected.

By building trusted relationships with service users, their whanau, and wider health and social service networks, we aim to empower people to achieve their health goals and enhance quality of life.

About you

To be successful as a Care Manager, you will hold a professional qualification and a current NZ Annual Practising Certificate (or professional equivalent) which has assessment as part of the scope of practice (e.g. Registered Nurse, Occupational Therapist, Physiotherapist, Speech Language Therapist, Social Worker etc.), as well as:

  • Have strong cultural awareness and commitment to implement strategies and services that are responsive to Maori and other high need populations.
  • Have significant clinical experience and display expert levels of clinical judgement.
  • Excellent written and oral communication skills.
  • Be confident in the application of policy and process.
  • Have high levels of computer literacy and ideally a knowledge of interRAI assessments.
  • Have sound knowledge and experience with assessment and care planning processes.
  • Be able to work autonomously and manage client caseload including the associated administrative /reporting procedures.
  • Be creative, solutions-focused focused and work in partnership to problem solve.
  • Have knowledge of a wide range of formal and informal local health and disability support services.
  • Be committed to ongoing quality improvement and high levels of service user and partner satisfaction.
