About the Role

We have an exciting opportunity for a Social Worker/Case Manager with Mental Health experience to join our Mental Health Services for Older People (MHSOP) Community Intake Team. The role will be part of our mahi of supporting Tangata Whaiora and their whanau in their journey to wellness.

You will work with the Tauranga community MHSOP services, as well as the Tauranga based inpatient MHSOP ward, health partners, referred clients, their families/whanau and the community to provide effective triage and assessment. You will facilitate and implement planned treatment to maximise health and independence in the community by:

  • Providing advice and assistance
  • Responding to and resolving mental health crises
  • Supporting smooth transitions along the care pathway for service-users and their families
  • Connecting consumers with primary care providers and appropriate community agencies

The focus is to deliver high quality care to the growing population in the Bay of Plenty. At the heart of our services are our CARE values - Compassion, All-one-team, Responsive and Excellence. This is a permanent 1 FTE position, working 40 hours per week.

About You

The successful applicant will be required to have an excellent understanding of mental health conditions, the Mental Health Act, PPPR Act, understanding of capacity assessment work up, a range of therapeutic modalities, dementia/delirium care.

The successful applicant will have a minimum of 3 years post graduate mental health experience, preferably with experience in older person's mental health
