Korero mo te turanga - About the role

Applications are invited from Registered Nurses to fill the position of two Diabetes Clinical Nurse Specialists (CNS). This is an exciting opportunity for a dynamic, experienced, and highly motivated individual. The Diabetes Clinical Nurse Specialist provides:

 •  Works as part of a Diabetes Integrated Team. Contributes to the development of integrated service delivery across the continuum of care in conjunction with medical, allied, and non-clinical support roles.
 •  Manages a case load of patients in a Diabetes Integrated Team (DIT) using a cluster model of care approach for community facing patient care.
 •  Rostered work in a variety of clinical setting including General Practitioner (GP) Services and Community providers (Tui Ora, Ngaruahine Iwi Authority, Ngati Ruanui Tahua Ltd and Pinnacle Midlands Health Network)

Coordinates services and patient clinical care
Develops evidence-based policies and procedures
Improves the experience for patients and their family/whanau

Mou - About you

Ideally candidates will have:

 •  New Zealand Registered Nurse with current Annual Practicing Certificate
 •  Four years post graduate practise and a sound knowledge base in Diabetes Management.
 •  Completed post graduate study in Long Term Condition Management, Diabetes Disease Management, or relevant papers.
 •  Designated nurse prescribing qualification (diabetes) or working towards.
 •  A strong focus on patient assessment.
 •  Strong leadership skills.
 •  Adaptability and flexibility.
 •  Motivation and committed to self-directed learning
 •  Excellent interpersonal skills.
 •  Be motivated and willing to learn about diabetes disease management in nursing.
 •  Ability to work within an integrated team
