Position Purpose:

As Pou Manaaki Clinician, you will lead and provide clinical oversight for WellSouth’s outreach services, including our inbound and outbound call centre operations. Your role is pivotal in ensuring best-practice outreach, proactive community engagement, clinical guidance, and effective liaison between WellSouth, health services, patients, and community providers.

Key responsibilities include triaging patients, updating, and maintaining accurate patient information across both internal and external patient systems, and referring individuals to the most appropriate health and/or social services. You will also be responsible for monitoring and supporting the performance of WellSouth’s outreach contracts, ensuring services meet clinical, cultural, and community needs.

This role is dynamic and responsive, adapting to the changing needs of the community and health system. The Pou Manaaki Clinician will work primarily with priority populations. A high standard of cultural competency and cultural safety is essential, ensuring that all interactions uphold mana-enhancing, equitable, and patient-centred care.

To be successful in the role you will:

  • Registered with the Nursing Council of New Zealand as a Registered Nurse, or other registered health professional.
  • Holds a current Nursing Council of New Zealand annual practising certificate, or a current practicing certificate.
  1. 5 years' experience in a clinical or relevant nursing role, demonstrating consistent professional practice and patient care.
  • Ability to deal with unexpected situations and circumstances while maintaining a professional manner.
  • Effective interpersonal skills.
  • Excellent written and oral communication
  • Good levels of computer
  • A high degree of self-motivation willingness, and self-responsibility to continually improve.
  • Strong time management skills and ability to work calmly
