We aim to support consumers that requireextended assessment or treatment following an admission to the acute inpatientservice prior to discharge.This may include:tāngata whaiorawho are presenting to services for the first time that may require extendedassessment and support around management of their symptoms.tāngata whaiorathat have an identified discharge pathway on admission
- i.e. home or supportedaccommodation
- but may require some additional assessment and upskilling tosafely and successfully transition into the community.
tāngata whaiorawho may have limited social supports and require support to increase theirengagement in their local communities.
The goals of the serviceinclude: Provision of asafe and supportive environment to provide extended assessment, treatment andrehabilitation following an admission to the adult acute serviceTeaching skillsto help support illness management and to support consumers to live the lifethey want to live Supportdevelopment of structure and meaningful routinesSupport safedischarge into a community settingPlease include a cover letter.About You We will be looking for experiencedRegistered and Enrolled Nurse, with mental health scope, who are passionateabout providing consumer centred care.
