Key responsibilities:As a Hauora Navigator, you will:Engage with patients and their whānau to build trust, understanding, and connection throughout their healthcare journey.Deliver the Whānau Hauora Assessment, supporting whānau to identify their strengths, needs, and priorities.Follow up on identified areas of need, ensuring timely referrals to appropriate services and providing education or resources to empower whānau in their care.Collect and record assessment and screening data accurately, maintaining high standards of confidentiality and data integrity.Support access and continuity of care by contacting patients for appointment reminders and assisting with coordination for Outpatient Clinics.Work in partnership with clinical teams, communicating effectively and escalating concerns to the Clinical Nurse Specialist or Charge Nurse Manager when appropriate Click here to view the position description.

About youThe ideal candidate will have:Experience in implementing Te Tiriti o Waitangi in action and practice.Basic understanding of te reo Māori me ona tikanga, and a willingness to deepen this knowledge.Excellent communication skills, with warmth and confidence to engage authentically with people from all walks of lifeGenuine empathy, confidence and cultural humility to build trust and connection with whānau in a mana-enhancing way.The ability to maintain confidentiality and handle sensitive information with integrityThe ability to work independently as well as part of a teamCompetence in Microsoft applications (Word, Excel, Outlook) and confidence with digital tools for recording and communication.
