Health New Zealand | Te Whatu Ora is firmly grounded in the principles of Te Tiriti o Waitangi and is committed to building a health system that serves all New Zealanders.About the RoleAn exciting opportunity has become available for an experienced and passionate New Zealand registered Allied Health professional in the area of Needs Assessment and Service Coordination (NASC).
As an Advanced Clinician, you will have strong communication skills and keenly understand the range of complex social & medical needs that a client requires in order to live a healthy, independent life.You will be able to:Apply advanced knowledge, skills, clinical reasoning and utilises evidence based practice when providing specialist intervention or caseload.Provide specialist consultation and clinical teaching based on contemporary best practice.
Provide clinical leadership to the NASC team through clinical research, audit and evaluation of service and client outcomes.Support the development of the wider Community Health Services by providing specialist advice and input into policy, planning and resources.Evaluate and implement service improvements for the NASC team.

About youAs an ideal candidate, you will:Have proven experience as a Needs Assessor/Service Coordinator (or in a similar role)Be fully trained in InterRAI assessment toolsDemonstrate strong clinical leadership in your day-to-day practiceBring over 6 years of post-graduate experience, including at least 2 years specifically in NASCConsistently set and uphold high professional standardsBe genuinely passionate about care coordination and patient navigationThis is a fantastic opportunity to work in a supportive team alongside a Nurse Specialist, experienced service coordinators and the wider MDT within a community approach to care.
