In this role you will carry a case load of clientele, supporting them through their recovery journey as part of, and with support from the wider team. The aim of this team is to work with referrers, health partners, families/whānau and the community to provide effective triage, assessment and management of Tangata Whaiora who are requiring our services.We work alongside Tangata Whaiora with the core values of patient centred care, and we achieve this with a focus on maximising health while promoting independence in the community. The focus is to deliver high quality care to the growing population in the Bay of Plenty.
In this role you will be working both independently and as part of the multi-disciplinary team. The successful applicant will be joining a multi-disciplinary team of Psychiatrists, Psychologists, Nurses, Social Workers and Occupational Therapists.
