This community clinical coordinator role will support both inpatient and community teams to coordinate efficient, effective and safe services, within available resources.This role is exciting and varied, it will:support teams to get people either hometo keep people living quality lives at homeprevent people presenting to hospital, and coordination of interim care into ARC facilities.You will work alongside:community teamsinpatient teamsARC facilitiesprimary care agenciesunit managersother clinical coordinators.This role provides leadership for the coordination of clinical service delivery, clinical leadership and direction to staff across community.In this role, you will:provide clinical leadershipmonitor and mitigate riskcoordinate ACC contractsactively engage in quality initiativesmanage the day-to-day functioning of the service to ensure delivery of safe patient focused caresupport staff education.This position does not encompass budget responsibility but has delegation of authority for management of staff within the community team.View position description.About the Team/Service/LocationHealth New Zealand
