Rates of pay are based on level of NZQA Health and Wellbeing Certificate qualifications

Level 0 = $23.73
Level 2= $25.15
Level 3 = $27.34

Level 4 = $29.54

There is also a travel reimbursement paid for travel between clients

Personal Specifications: A Community Support Worker will be a person of high personal integrity with a commitment to
the care of the elderly and have the ability to:
  • Communicate effectively
  • Enjoy good interpersonal relationships with all staff, clients and their families
  • Be able to work independently.
  • Work under the supervision of the Community Registered Nurse, Community Care
Coordinator, Nurse Manager.
  • Meet the changing needs of Clients and Agecare Central Ltd business.
Qualifications:
  • A current First Aid Certificate.
  • Current Drivers license and own vehicle
Responsible to:
  • The Community Registered Nurse
  • The Community Care Co-ordinator.
  • Nurse Manager
Functional Relationships, Communication Liaison with:
  • Chief Executive
  • Nurse Manager Clinical Manager
  • Clinical Co-ordinator & Registered Nurses
  • All Staff
  • Health & Safety Committee Representatives
  • Clients and their families
  • Visitors and members of the public
  • Allied Health Professionals
Primary Objective:
  • To deliver high quality community care in accordance with the referrals/care plans as directed

by the Community Support Coordinator/ Community RN.

Key Task 1

To effectively communicate with staff, Clients and families.

Performance Indicators
  • Encourage and support open channels of communication between staff, clients and their
families within the limitations of responsibility and authority of the role.
  • Report promptly to Community Care Coordinator/Community RN any change in Client
condition.
  • Accurate and timely reporting, and documentation in clients progress notes.
  • All client related care/concerns/appointments are communicated effectively through a variety
of channels e.g. telephone, concerns form, accident incident forms, directly to the Community

Care Coordinator, staff meetings.

Key Task 2

Quality care is delivered according to the support plan meeting individual physical; emotional, spiritual and cultural needs.

Performance Indicator
  • Care is delivered in a professional manner; upholding and advocating the Health & Disability
Code of Rights and Elder Protection Policy at all times.
  • Safe delivery of care is guided by compliance to the support plans which outline Support
Worker tasks and responsibilities.
  • Support workers will practice safely within the procedures and policies of Agecare Central.
  • Support workers delegated to carry out client specific clinical procedures will undergo
competency based assessments as required.
  • Delivery of care will be cost effective and reflect current best practice.
Key Task 3

To take responsibility for Health & Safety/Infection Control.

Performance Indicators:
  • Compliance of Health & Safety/Infection Control policies and procedures.
  • Identify, report and document any potential/significant hazards relating to staff, clients
environment, equipment or maintenance.
  • Prompt reporting and documenting of any staff, client, and visitor accidents/incidents.
Key Task 4
Commitment to Agecare Central Quality Assurance Programme
Performance Indicators
  • Participation/input into community support worker meetings and other company open
meetings e.g. Health and Safety meetings.
  • Provide input/feedback on reviewed, draft or newly developed protocols; Task lists
  • Identify deficits and provide recommendations for continuous quality improvement.
Key Task 5.
Commitment to ongoing Professional Development
Performance Indicators
  • Attendance at compulsory education sessions
  • Attendance at relevant in-service/training.
  • To take some accountability for self-directed learning .
  • Identify own training needs in conjunction with NM/CM and Community Care Co ordinator
  • Provide input into staff yearly training/education programme
Key Task 6:
To protect and care for the business of Agecare Central Ltd:

Performance Indicators: Compliance of the Code of Conduct and Confidentiality

Duties will be carried out in a professional manner.
The appropriate reporting channels will be used to report any objective observation which may
contribute negatively toward a client, visitor or staff’s wellbeing at which
could violate the integrity of Age Care Central Ltd in any way.
Limitation on Authority

Must work at all times within the Policies and Procedures of Agecare Central Ltd
