Salary: $88,627 - $110,210

The current Social Worker is leaving the country, so a fantastic opportunity is available to join a dynamic team of health professionals at The Front of Whare at Wellington Hospital in the Emergency Department and the Medical Assessment and Planning Unit.

This is a full time, permanent position working with patients aged 16 years and above.

Our Front of Whare team is evolving and now is the chance to be part of the change. We're developing from a multidisciplinary model of care to grow an inter-professional allied health team. Working collectively to address unplanned care with a patient focussed, problem solving approach, this team is all about pace and agility whilst maintaining high clinical standards.

You will work with a diverse group of patients with varying health needs. You will complete psycho social assessments with patients and their whanau to identify their support needs in regards to their health and discharge planning. Part of this role is working with patients who are experiencing Family Violence so the social worker will undertake a risk assessment and safety plan to address these concerns and utilise supports from external agencies.

You may need to work Saturday's on a roster basis providing a service to the inpatient services.

About the Team/Service/ Location

We are a diverse and inclusive team of 23 social workers who provide a service to patients and their whanau across inpatient, outpatient and community settings. We complete psycho social assessments across the age range and provide holistic social work interventions.

We encourage therapeutic social work and trauma informed practice, and value patient centred care. The role will be well supported by the other medical social workers and social work coordinator in the team. Come and join our friendly experienced team in a supportive workplace environment.

About you

As the successful candidate you will have a passion for working in this area and be highly motivated to support patients and whanau navigate their health journey. You will be able to work collaboratively and effectively as part of a multi-disciplinary team and with external agencies.

Working with a diverse group of patients with significant health needs the post holder will require excellent documentation, communication and time management skills. This is a great opportunity for someone who is keen to work in this clinical area.

The successful applicant will have the following skills and attributes:

  • NZ Registered Social Worker with current annual practicing certificate
  • Minimum of 3 social work practice, though will consider those with less experience
  • The ability to complete psychosocial needs assessment
  • Navigating and working within the legal requirements of health care and social work
  • A clear understanding of the health needs of Maori and Pacific Island communities
  • Therapeutic social work counselling experience (desirable)
  • Previous work in a health setting and a good understanding of health social work (desirable)
