About the RoleA position has become available in the Community Allied Health Services (CAHS) Team in Tauranga for a permanent full-time Social Worker with at least three years' experience working in a health-related setting. This role involves the provision of social work services to clients within their own homes and community.
The position is based at the CAHS office in central Tauranga and covers a wide geographical area in the Western Bay of Plenty. The Allied Health team works alongside individuals with physical health needs using a client centred, enablement approach.
The team's focus is to maximise client safety and functional independence; to enable them to achieve their wellness goals so they can remain living in their own home. A key part of this role is establishing and utilising professional networks, then using these networks to problem solve and meet client's needs.
Another is building a knowledge of health systems and services, then using this knowledge to assist clients to navigate these systems and services. About the Team/Service/LocationThe CAHS team is comprised of allied health assistants, dietitians, occupational therapists, physiotherapists, social workers and speech language therapists.
