This role sits within the Needs Assessment and Service Coordination team in the Adult Community Mental Health and Addictions Service in Tauranga.As a Registered Mental Health Professional, you will work alongside Tāngata Whaiora and their whānau to develop personalised recovery plans and coordinate the community resources needed to support independence and wellbeing.

The role involves comprehensive needs assessments, collaborative care planning, and coordination of community support services. Strong partnerships with specialist clinical teams, support providers, housing services, and other community agencies are central to achieving positive outcomes.This position also offers opportunities to contribute to service development initiatives, including supported housing approaches and the implementation of shared care pathways across clinical and community services.About the Team/Service/LocationThee Needs Assessment and Service Coordination team forms part of the Adult Community Mental Health and Addictions Service in Tauranga.

This small, dedicated team works closely with clinical services, community providers, and government agencies to ensure people receive the right support at the right time.
