OamaruSalary band - $77,087 - $110,210 per annum pro-rata and KiwiSaverPermanent 0.8 FTE (32 hours per week) opportunityAccess to regular supervision, professional development, and peer supportHealth New Zealand | Te Whatu Ora is firmly grounded in the principles of Te Tiriti o Waitangi and is committed to building a health system that serves all New Zealanders.About the roleOur Needs Assessment team is passionate about the community it serves and works effectively, keeping our clients' best interests at the top of our minds.You will be joining a hardworking team and will play an important part in its successAssess people who are predominantly over the age of sixty-five and need support because of agingAssess clients in their homes and in a variety of health settings for example hospital, hospice, aged care facilitiesUtilise the national assessment tool interRAI to assess and determine best outcomes for our client groupThe assessment is a structured, comprehensive electronic tool that you will work through with the client in a conversational mannerWrite comprehensive reports in digital systemsCreate a plan of care that is client centred, and goal focused including developing support packages in a holistic and culturally appropriate manner for older people who require long term support in the communityPlay a key role in connecting people and their whānau with formal and informal support services to assist them to achieve their goalsView position descriptionAbout you:Registered health professional with a current practising certificateYou will have experience working with older people in either a hospital, community, or aged care environmentYou will need an excellent knowledge of community resources and services that are available to the older personIt is essential that you have a current NZ driver's licenceCompetent with computers and able to perform the duties listed above to a high levelSalary rangeThis role is covered by the terms and conditions of the Allied, Public Health, Scientific and Technical CEA and the salary will be in accordance with the pay scale, between Step 1 $77,087 and Step 8 $110,210 per annum pro-rata and the NZ Nurses Organisation Nursing and Midwifery CEA Thursday, 31 March 2023 - Thursday, 31 October 2024, and the salary will be in accordance with the pay scale, between step one $75,773 and step seven $106,739 per annum pro-rata depending on relevant experience.Working at Health New ZealandHealth New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
