Community Lead, Supported Transition and Restorative Team (START)Health New ZealandWaitemataPermanent, Full time position Waitematā District is developing an exciting new interdisciplinary Service to support patients restore functional abilities in the community - the Supported Transition and Restorative Team (START)As START Community Lead you, with the START Manager, will develop and lead the standards and direction of the Service.
You will manage a team of registered clinicians (allied health and nursing) supported by Allied Health Assistants to deliver tailored, short-term restorative interventions to patients in their place of residence. As a registered Nurse or Allied Health professional you will have the opportunity to demonstrate your professional clinical and leadership skills with a focus on restorative practices in the community.

As the Community Lead you will be responsible for the day-to-day management of the community focussed side of the new service. You will provide clinical leadership ensuring patient safety by monitoring standards of care and best practice.You will lead, and be responsible for, the community interdisciplinary team comprising allied health and nursing healthcare practitioners, assistants, and administrators to provide quality support and restorative care to facilitate patients achieving their functional goals.You will effectively contribute to the development and implementation of service business plans in line with Health NZ Waitematā values and strategic direction, ensuring that both current and future demands are met.About the Team/Service/ LocationSitting in the Specialty Medicine and Health of Older People (SMHOP) Division the START team will support patients across the Waitematā District, based initially in two community hubs of North Shore Hospital and Waitakere Hospital delivering support services in the community.For more information on living and working in Waitematā click hereAbout youTo be considered for this role you must meet the following criteria:Hold a current Annual Practising Certificate in any of the following:New Zealand Nursing CouncilOccupational Therapy Board of New ZealandPhysiotherapy Board New ZealandPossess highly developed clinical and professional skills and knowledge (at least 5 years professional experience)Demonstrate strong leadership and management abilitiesBe able to lead a team operating in the communityHave experience in rehabilitation and restorative practiceHold a full New Zealand Driver’s LicencePlease click here for position descriptionWorking at Health New ZealandHealth New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
