Working autonomously within a multidisciplinary team alongside a geriatrician and pharmacist, you will provide early assessment, diagnosis and treatment of acute deterioration, support goals-of-care discussions, and coordinate care to reduce avoidable Emergency Department presentations and hospital admissions.
