About the Role This is a Community based role in the Hutt Valley. The Social Worker in this position will be working with IDT colleagues to understand the relationship between diagnoses, the impact on function and the range of what might be required from a Psycho-social perspective to move recovery forward or assist someone to live sustainably with long-term disability/health conditions.

Our clients require support with housing/homelessness, financial concerns, family conflict/abuse neglect, legal and capacity issues etc. You will support our clients and whānau to navigate the systems and access the resources they require, whilst also promoting self- management and self-responsibility as appropriate.
About the Team/Service Location Te Herenga Tangata is a large interdisciplinary team including not only Social Workers, but Occupational Therapists, Physiotherapists, Nurses, and Assistant staff. We work closely with medical staff specializing in older adults and care for mid age people with complex health conditions.
Our team also has close links with our Dietetic and Speech Language therapy colleagues, and we are continuing to build relationships in primary health care. We have a key role in preventing unnecessary acute presentations by proactively assisting people with health conditions to remain stable at home.
We are applying systems to assist caseload management and patient flow to optimise our ability to pick up referrals from the community and post discharge from hospital in a timely way.The team is continuing to develop inter-professional practice approaches so (following the relevant training), you'll also have the opportunity to engage in tasks that will relieve other professionals within the team of unnecessary interventions.
About you We are working with people in challenging situations so the ideal candidate will;Have experience in Community based Social Work within the New Zealand public health system Enjoy working in an IDT and be willing to engage with complexity.
