About the Role
The District Nurse is responsible and accountable for assessing, planning and implementation of programmes of care. These will be developed in partnership with patients, family, whanau/carers and significant others and will be evidence based taking into consideration lifestyle, race, gender and cultural background and factors which impact on physical, mental or emotional health.The District Nurse works in partnership with patients and their families/whanau and collaborates with the multidisciplinary team in both the primary and secondary health settings and recognises and supports the delivery of Health New Zealand Te Whatu Ora Lakes vision and demonstrates the values in the workplace.
As a District Nurse you will experience daily visits to patients. A typical day includes an early start, performing scheduled visits, responding to messages, conducting home-based treatments, and finishing with patient checks and team communication.Incorporated in patient visits is the education and treatment of patients and families, documentation of care, response to calls and collaboration with the wider healthcare team to provide wholistic care for individuals and families.
About the Team/Service/ Location
A typical district nursing team incorporates Registered Nurses (RNs), nursing assistants, alongside the client and their family, using generalist and specialist skills to provide holistic care in the home and community. The team emphasizes a restorative, socio-ecological approach, recognizing social determinants of health, and functions as an intermediary between hospital and community care, providing transitional care and enabling self-care.

About you
As a District Nurse you will:
- Hold New Zealand Nursing Council registration (NCNZ).
- Hold a current Practicing Certificate
- Hold a current drivers licence for both manual and automatic vehicles.
- Have evidence of practicing at Level 3 of an approved PDRP programme.
- Have at least 4-5 years post graduate nursing experience in an acute medical/surgical setting.
- Possess Advanced IV management (CVAD), Port-A-Cath, PICC, Midline experience
