About the RoleWe have an exciting opportunity for a mental health nurse wishing to advance their career within Mental Health and Addictions Service.The clinical nurse coordinator will work in close collaboration with Awhi Mai and Adult Mental Health Services.

Awhi Mai is a joint Kaupapa Maori service initiative between Te Awhi Whanau Charitable Trust and Adult Mental Health & Addictions Service, Waikato DHB. Maori and non-Maori can be referred.The service is designed to provide care for Tu Whanau (tangata whaiora / service users) as an acute alternative to the Henry Rongomau Bennett Centre (HRBC) inpatient unit or Tu whanau from the community.As the clinical nurse coordinator you will assess all referrals for suitability to be admitted to Awhi Mai and liaise with the rest of the clinical team and the team at Awhi Mai.

You are required to also monitor and manage Tu Whanau mental status and liaise with whanau, and other outside agencies.This role enables you to work across a variety of settings and great autotomy.Responsibilities of the role:Assess, maintain and manage clinical risk.Build relationships with tāngata whaiora, whanau and support agencies.Conduct thorough assessments of tāngata whaiora health status and develop appropriate care plans in partnership with them.Collaborate with other healthcare professionals to ensure the best possible outcomes for tāngata whaiora.Commit to the MDT approach with a focus on excellent service delivery.

About the TeamOur small team of clinicians made up of a Doctor, Nurse and Occupational therapist provide expert assessments and ongoing treatment with Tanagata whaiora with major mental illness. We work closely with the tanagata whaiora, their whanau and also with te awhi whanau charitable trust to ensure the best outcomes for our tangata whaiora.
