About the Role
As the 'Winter Boost' Clinical Needs Assessor, you will be part of the 'SSHOP Winter Boost team. You will be working alongside our Nurse Practitioner Health of Older People and Gerontology Clinical Nurse Specialist to follow up complex and frail older adults who have presented to ED and would benefit from a comprehensive geriatric assessment at home.You will be working closely with the wider community services team and be making best practice recommendations to their GP to support the older adult being able to remain living well in their community of choice.
This is a fixed term full-time role for five months, working up to 80 hours per fortnight (1.0 FTE), Monday to Friday between the hours of 7.00AM to 7.00PM. Please state your preferred FTE in your application as this position can be shared between more than one applicant.

About you
As a Clinical Needs Assessor, you will:
- Be a motivated and enthusiastic individual, ideally with experience working with individuals with disabilities;
- Have knowledge and understanding of disability models of care and support desired;
- Hold a full driver's licence.
We are looking for a person who:
- Has experience working with individuals with disabilities;
- Has the ability to work in both a team environment as well as independently;
- Is flexible and adaptable.
- Preference is given for applicants with an allied health background.
The successful candidate must be eligible to work in New Zealand or hold a valid NZ visa at the time of applying.
