Registered Nurse - Care Co-ordinator
Home SWEET Home Care is looking for a Registered Nurse to co-ordinate care and oversee our team and clients in the Manawatu.
About the Role:
- Enjoy the autonomy of assessing new clients in their homes, using your excellent clinical skills.
- Collaborate with the Care Manager to develop tailor-made care plans that suit individual needs.
- Ensure ongoing quality care by overseeing a team of dedicated caregivers matched to each client.
- Being available to answer Carer queries as needed.
- Work on a casual basis, perfect for those seeking additional hours to complement existing part-time work.
Home SWEET Home Care is a growing service originally based in Whanganui, and now in the Manawatu and Wairarapa.
Our team has a boutique feel, with high input from the Director/Care Manager who is readily available to support both our valued clients and the team of Carers and Registered Nurses. Our service works alongside the funded services that might already be in place, providing additional support that may be needed to allow our clients to stay in their own homes for longer.
The Registered Nurse will have an understanding of how they will support the Carers to carry out some of the following care to our clients. These visits are from a minimum of 2 hours and up to 24 hour care if needed.- Personal care - showering or wash, dressing and grooming
- Companionship - spending quality time at home, outings, support at medical appointments, walks or activities
- Dementia care, palliative care, respite care, post surgery or hospitalisation support
- Home management - may include light housework, meal preparation and laundry
Requirements:
- Current New Zealand Registered Nurse Practicing Certificate.
- Availability to assess clients when referrals come through.
- A passion for improving lives and delivering compassionate, client-centered care.

Ideal candidate must have the following:
- Current practicing certificate
- Must have 10 years plus experience, with some palliative care experience preferred
- Excellent communication and leadership skills, and excellent presentation
- Full NZ drivers licence and a clean, reliable roadworthy car
- Clean police check
- Able to be flexible with doing client assessments when required, and oversight of the clients care.
- Must currently live in New Zealand
