As a Care Manager within the Community Services Group, you will carry out comprehensive assessments, develop person-centred care plans in partnership with whānau, and coordinate support packages to ensure individual needs are met and regularly reviewed.This role involves working closely with service users and their whānau to promote self-management wherever possible, while also collaborating with hospital-based clinicians, community services, and other providers to deliver seamless, wraparound support.
You'll need to be relationship-focused, organised, and detail-oriented, with the flexibility to operate across a range of settings, including people's homes, hospitals, hospices, and residential care.Based at either Taranaki Base Hospital or Hāwera Hospital, there is also scope for some remote working.A full NZ driver's licence is essential due to the community-based nature of this role.For information on the salary band and the position description, please click here.
