About the Role

As a Needs Assessor, you will:

Play a critical role in evaluating and coordinating care plans for older persons experiencing health challenges or requiring support to maintain their independence.

Work collaboratively with individuals, families, and multidisciplinary teams to ensure that clients receive tailored support aligned with their health and wellbeing goals.

Make a meaningful impact in the lives of older adults while working in a supportive and collaborative environment

This is a permanent part-time position based in Richmond working 48 hours per fortnight (0.6 FTE), Monday to Friday. No on-call is required.

About the Team/Service/ Location

You will have close working relationships with the Wairau Needs Assessment team at Wairau Health Hub, and will work in partnership with Te Piki Oranga Maori Health Providers, community services, inter-district health services, and Iwi.

About you

We are seeking an enthusiastic and experienced Needs Assessor who is either a registered Occupational Therapist, Social Worker, Physiotherapist, or Registered Nurse with experience working with older adults.

The successful candidate will:

  • Conduct comprehensive assessments of clients' health, social, and functional needs in line with government guidelines.

  • Develop, implement, and review personalised care plans in collaboration with clients, families, and healthcare providers.
  • Work closely with multidisciplinary teams to ensure appropriate services and support are provided.
  • Advocate for clients, ensuring their needs are prioritised and their voices heard.
  • Maintain accurate documentation and reports in compliance with relevant policies and standards.
  • Provide education and guidance to clients and families about available resources and support systems.

The successful candidate must be eligible to work in New Zealand or hold a valid NZ visa at the time of applying AND have a clean NZ Driver's license.
