Tauranga BasedPermanent, full-time position About the RoleWe have an exciting opportunity for a Social Worker/Case Manager with Mental Health experience to join our Mental Health Services for Older People (MHSOP) Community Intake Team. The role will be part of our mahi of supporting Tangata Whaiora and their whānau in their journey to wellness.You will work with the Tauranga community MHSOP services, as well as the Tauranga based inpatient MHSOP ward, health partners, referred clients, their families/whānau and the community to provide effective triage and assessment.
You will facilitate and implement planned treatment to maximise health and independence in the community by:Providing advice and assistanceResponding to and resolving mental health crisesSupporting smooth transitions along the care pathway for service-users and their familiesConnecting consumers with primary care providers and appropriate community agenciesThe focus is to deliver high quality care to the growing population in the Bay of Plenty.At the heart of our services are our CARE values - Compassion, All-one-team, Responsive and Excellence.
