We have a great working relationshipwith District Nursing, Complex Clinical Care Network, Rural Nurse Specialists,and other community services across the West Coast.
The role also links closelywith our hospital multidisciplinary team ensuring the transition from home tohospital and back home again is well supported.As a Social Worker you will beresponsible for providing appropriate and effective psychosocial assessment,intervention, monitoring and evaluation to clients.Whilst no day is the sametypically you will be in the community alongside other physical and mentalhealth professionals with clients of all ages.
