This role will provide patient-focused care and coordination that will optimise the patient journey and improve health outcomes.About the Team/Service/LocationThe Community Mental Health service provides community-based assessment and treatment services to people, including those presenting with alcohol and drug issues, through both community follow-up and outpatient clinics.

This service also provides the crisis response for mental health, dual diagnosis and clinical rehabilitation.For more information on living and working in Northland click hereAbout youWorking with a passionate, multi-disciplinary team who place great value on putting people first, you will have the standard of clinical practice and expertise necessary to effectively provide for tāngata whaiora and whānau needs within the service here in Northland.Key requirements:New Zealand registered Nurse Practitioner with a current practicing certificateExperience in mental health nursing, preferably in a community settingStrong communication and interpersonal skillsAbility to work effectively within a multidisciplinary teamCurrent full driver's licenceKnowledge and/or experience working with people who have coexisting mental health and substance use problemsWorking at Health New ZealandHealth New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
