Social Worker - InpatientPermanent, Part-time (48 hours per fortnight) Monday to FridayRotorua HospitalHealth New Zealand | Te Whatu Ora is firmly grounded in the principles of Te Tiriti o Waitangi and is committed to building a health system that serves all New Zealanders.We are looking for a registered social worker who is:Compassionate, adaptable and able to work in a fast- paced hospital environment.Skilled at connecting and building rapport with patients and whanau.Confident working within a multidisciplinary team.Committed to upholding Te Tiriti o Waitangi and promoting equitable health outcomes.Seeking a rewarding, challenging role with the opportunity to make a difference.About the RoleAs an inpatient social worker you will:Provide psychosocial assessments, interventions and discharge planning in collaboration with the wider multidisciplinary team.Identify psychosocial risk factors such as family harm, elder abuse and child protection.
The social worker will formulate safety plans in collaboration with patients, whanau and the clinical team to ensure safety during and after discharge.Support patients and whanau to navigate complex health journeys and discharge planning.Advocating for patient’s rights, equity and access to resources.Collaborating with multidisciplinary teams to ensure safe and timely discharges.Actively participate in team meetings and contribute to care planning and discharge co-ordination.Engage in quality improvement initiatives and professional development activities.
