About the Role We have an exciting opportunity for a motivated and experienced nurse to step into a leadership role as Associate Charge Nurse Manager (ACNM) Wellington Community Health teams.As ACNM, you
  • ll play a key role in supporting acute flow and facilitating safe, timely discharges from hospital to home.
You
  • ll work closely with District Nurses, Enrolled Nurses, and Health Care Assistants, ensuring referrals are actioned efficiently, workloads are balanced, and high standards of care are maintained.This role also includes:Supporting Early Supported Discharge (ESD) serviceOverseeing rostering, recruitment, and health & safetyAssisting with performance reviews and professional developmentContributing to service improvement and strategic planningAbout the Team/Service Location Our Wellington District Nursing team is based at the Ewart Building, 2 Coromandel Street, Newtown, and works closely with the Kapiti and Kenepuru teams.

  • re looking for a senior nurse with:Proven leadership experience in clinical and professional settingsA passion for community-based care and improving patient outcomesStrong prioritisation and decision-making skillsFlexibility and adaptability in a dynamic environmentCurrent Annual Practicing Certificate as a Registered Nurse with the NZ Nursing Council.Previous experience in a nursing management position and experience/interest in working in the community setting.Broad range of experience in clinical nursing experience of at least five years.Working toward a post grad qualification.Able to develop and maintain relationships with other community healthcare providers.Ability to work collaboratively.The position is fill time (1.0 FTE), working 5 days a week, 0800am to 16.30pm, and includes weekend ACNM cover.Working at Health New Zealand Health New Zealand | Te Whatu Ora is
