You will work with Tauranga community MHSOP services, as well as the Tauranga based inpatient MHSOP ward, health partners, referred clients, their families/whānau, and the community to provide an effective triage and assessment, to facilitate and implement planned treatment to maximise health and independence in the community:Provision of advice and assistanceResolution of the mental health crisisFacilitating transition along the care pathway for service-users and their familiesLinking consumers to primary care providers and appropriate community agenciesWe have a commitment to working in partnership and collaboration with consumers, family/whānau and the community.About YouThe successful applicant will be required to have an excellent understanding of mental health conditions, the Mental Health Act, PPPR Act, understanding of capacity assessment work up, a range of therapeutic modalities.The successful application will have a minimum of 3 years post graduate mental health experience, preferably with experience in older person's mental health.Working for Health New ZealandHealth New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
