Registered Mental Health Nurse

About the Role The Intake andAssessment Clinician's (Nurse) role facilitates access to mental healthservices through providing an initial triage and assessment service and thenreferring to the specialist community mental health team or to other servicesas appropriate.Key priorities ofthe role include:Triaging referrals from a variety ofsources in the community.Undertake face to face initialassessments.
Recognise that culturally safepractices are paramount and liaise with relevant services to ensure peoplereceive safe, high quality and equitable care.Assess and escalate any crisis/urgentreferrals to the appropriate crisis mental health and addiction services.Works collaboratively alongside thecommunity mental health teams and local service providers to access the mostappropriate service/support for the person based on their requirements.Liaise with referrers, including localGP's and community networks/NGO providers in the region to ensure people getappropriate, timely and person centred-care and feed back to them if there areany issues regarding quality of referrals About the Team/Service/ Location Hutt SouthCommunity Mental Health Services is the largest community based mental healthteam in Wellington, serving a diverse geographical area and a diversepopulation.
