The Hereford Centre provides specialist mentalhealth support to individuals experiencing complex, severe, and enduring mentalhealth challenges. Our Assertive Outreach approach works specifically withpeople who have historically found it difficult to engage with conventionalcommunity mental health services and who may be at increased risk of relapse oradmission to inpatient care.
The team is currently otherwise fully staffedand holds a high level of clinical expertise. We are seeking a motivated andpositive Registered Nurse to contribute strong clinical skills and workcollaboratively within a supportive multidisciplinary team.
About YouWe are looking for a professional and highlymotivated nurse to join our team. Interacting with a variety of stakeholders, you will have anability to effectively manage and prioritise a caseload in a case managementposition.You will work within a multi-disciplinary team, and in collaboration with our tāngata whaiora and their family/whanau, primary care, community supports and our inpatient teams.It is essential that you have the ability to think on your feet, triage and prioritise your tāngata whaiora's needs appropriately, and in a timely manner.You will case manage a caseload of tāngata whaiora who you will proactively engage with and support to engage with their treatment and other holistic supports.Highly developed risk assessment is important as is your ability to develop effective rapport with people who may be mentally unwell or distressed.Assessment, development of treatment plans and liaising with whanau and other carers, NGOs, police, probation and other services is all part of the Community Mental Health Nurse role.You will haveEssential:Registered with the Nursing Council of New Zealand with a current APC and mental health scopeIdeally, substantial experience working within community mental health and a sound understanding of case management within this context.Have a full New Zealand driver's licenseKnowledge of and a commitment to Te Tiriti o WaitangiExcellent interpersonal skills and relationship buildingThrives in a fast-paced environment in a varied roleTo be successful in this role, you will need to bring substantial experienceDesired:Have a clinical background in addressing substance abuse.Duly Authorised Officer (DAO) status would be a significant advantage.

If the successful candidate is not a DAO then we would expect that they work towards picking up the role within the first year with the Hereford Centre.Working alongside the equally well initiative the successful candidate will prioritise the physical wellbeing of our whaiora and either bring the skill or be willing to learn taking ECGs, venepuncture and general physical health recordings.
