We value respect andintegrity and endeavour to provide a high quality and accountable service.This role provides Social Work Services todesignated areas of work within Older Persons Health and Rehabilitation in bothour outpatient and inpatient settings.Engage with patients and theirwhānau to complete biopsychosocial assessments and develop tailored care plans.Provide emotional, practical,and informational support, advocating for patients and linking them toessential services.Respond to health and disabilityneeds that impact on the wellbeing of the individual and their family/whanau.Communicate effectively withpatients, whānau, and multidisciplinary teams to enable collaborative decision-making.Maintain accurate, timely, andconfidential records using digital systems.About YouWe are seeking a compassionate Social Worker witha strong interest in Older Persons Health and Rehabilitation.
To succeed inthis role, you will have:A flexible and adaptableapproach to your workSWRB registration and a currentAnnual Practicing Certificate2+ years' experience in hospitalSocial Work (desirable) A robust understanding ofhospital processes and proceduresExperience in implementing TeTiriti o Waitangi in actionStrong digital skills The ability to work underpressure, manage stress, and respond effectively in crisis situations.Exceptionalrelationship-building and advocacy skills, with a commitment to supportingvulnerable patients and whānau.Competency with te ao Māori,tikanga, and te reo Māori and a commitment to continuing your journey andtaking ownership of your learning and growth.Working at Health New ZealandHealth New Zealand | Te Whatu Ora is dedicated to ensuringexcellent healthcare for the people of New Zealand.
