About us:Te Hau o Te Ora Invercargill and Mataura are two small practices with a combined enrolment of 3,600 patients. The practices are supported by highly skilled nursing workforce, Health Improvement Practitioner, Health Care Assistant, Kaiawhina and a large administrative team.

We are seeking a Health Coach to join our team at Te Hau o Te Ora, you will be working across both Invercargill and Mataura.
The role:Health Coaches are based in general practice and support people by taking positive steps towards improving their health. This includes:
- Empowering patients to take control of their health and wellbeing
- Helping patients develop realistic goals
- Encouraging good management of various health issues that patients want addressed
- Discuss how to be more active by doing things they enjoy
- Providing support to people and their whanau in their own community
- Supporting independence
- Connecting to services in the community
- Working with people towards their health and wellbeing goals.
- Strong cultural confidence and competence, keeping whanau at the centre of care provided.
- An understanding of the socio-economic and cultural determinants of health.
- Be self-motivated and independent yet thrive in a collaborative environment.
- Whanaungatanga: strong community relationships to enable holistic whanau-centred support.
- Excellent communication and inter-personal skills.
- Prior experience with Medtech Evolution desired but not essential.

You will be well supported in this role, initially receiving training through Health Literacy alongside a comprehensive 3-month induction to help you fully integrate the role into the practice.

- Be legally able to work in New Zealand,
- Hold a full New Zealand drivers’ licence,
- A current first aid certificate or be prepared to obtain one
- Be required to undergo pre-employment screening,
