We are committed to providing high-quality, patient-centred care, and our Heart Failure Service is a key part of our commitment to managing chronic health conditions effectively in the community.We are currently seeking a Clinical Nurse Specialist (CNS) – Heart Failure to join our dynamic and compassionate team.
This role offers an exciting opportunity to provide expert care for heart failure patients in their community settings, improving patient outcomes and contributing to the sustainable management of heart failure.As a Clinical Nurse Specialist (CNS) in Heart Failure, you will play a pivotal role in the holistic management of heart failure patients and their whanau, within the community.
You will work closely with multidisciplinary teams to provide expert advice, patient education, and ongoing care. Your work will help reduce readmission rates, enhance medication adherence, and improve the quality of life for those living with heart failure.
Key Responsibilities:Provide specialist heart failure nursing care to patients, including titration of medications, symptom management, and education.Lead the delivery of heart failure care in the community, ensuring continuity of care post-discharge.Conduct comprehensive assessments and develop personalised care.Work collaboratively with cardiologists, GPs, and other healthcare professionals to ensure best practice and coordinated care.Educate patients and their whanau on heart failure management, lifestyle changes, and self-care.Monitor and evaluate patient progress, adjusting care plans as necessary.Contribute to quality improvement initiatives and support research in heart failure care.Click here to view the position description.
About the Team/ServiceOur dynamic Heart Failure Nursing Team delivers expert, community-based care designed to optimise heart function, reduce hospital admissions, and empower patients to live healthier, more independent lives.We work at the heart of a fully integrated system, building strong partnerships with both primary and secondary care to ensure seamless support.Every day, we place patients and their whanau at the centre of our work, delivering personalised care with compassion and clinical excellence. About you:To be success in this role you will need:Registered Nurse with a current New Zealand practicing certificate.Postgraduate qualification in cardiology or a related field is preferred, or a strong commitment to ongoing professional development.Minimum of 5 years’ experience in heart failure management or a relevant clinical setting.Advanced clinical assessment and nursing skills.Excellent communication and interpersonal skills, with a focus on patient education and empowerment.Ability to work independently and as part of a multidisciplinary team.Knowledge of New Zealand healthcare systems and services, including DHBs and primary care.
