Based at Whakatāne Hospital, Fixed term 12 months - 2 FTE - 2 Positions availableSubsidised onsite hospital gym membership + Free Flu vaccinations + Discounted goodsHealth New Zealand | Te Whatu Ora is firmly grounded in the principles of Te Tiriti o Waitangi and is committed to building a health system that serves all New Zealanders.About the RoleBay of Plenty - Hauora a Toi are seeking experienced Allied Health Assistants, Rehabilitation Assistants or Health Care Assistants to participate in providing care and therapy under delegation from registered health professionals in a new service called START (Supported Transfer and Accelerated Rehabilitation Team).
START is an early supported discharge model of care in which older people can return to their home following an acute episode or injury to complete their rehabilitation/ enablement at home. The care is provided by trained health care assistants or support workers.The position involves shift work some of which will be weekend work and out of standard office hours.About the Team/Service/ LocationThe START team will aim to reduce the risk of readmission to hospital through:Improving functional resiliency / independenceEnsuring seamless re-integration back to primary careManaging issues related to medication, nutritionManaging carer stressIdentifying deterioration in conditionsSupervising, managing care packagesThis position is based in Te Koru rehabilitation department at Whakatāne Hospital, with most of the work in the Eastern Bay of Plenty community supporting patients' rehabilitation and enablement in their own environment.
